How To Get Vaccinated Against Hepatitis B

All babies in the UK born on or after 1 August 2017 are given 3 doses of hepatitis B-containing vaccine as part of the NHS routine vaccination schedule.

Full protection involves having 3 injections of the hepatitis B vaccine at the recommended intervals.

Babies born to mothers with hepatitis B infection will be given 6 doses of hepatitis B-containing vaccine to ensure long-lasting protection.

If you’re a healthcare worker or you have kidney failure, you’ll have a follow-up appointment to see if you have responded to the vaccine.

If you have been vaccinated by your employer’s occupational health service, you can request a blood test to see if you have responded to the vaccine.

Hepatitis B Vaccination In Pregnancy

Hepatitis B infection in pregnant women may result in severe disease for the mother and chronic infection for the baby.

This is why the hepatitis B vaccine is recommended for pregnant women who are in a high-risk category.

There’s no evidence of any risk from vaccinating pregnant or breastfeeding women against hepatitis B.

And, as it’s an inactivated vaccine, the risk to the unborn baby is likely to be negligible .

If a virus is not fully wiped out during a regimen of antivirals, treatment creates a bottleneck in the viral population that selects for resistance, and there is a chance that a resistant strain may repopulate the host. Viral treatment mechanisms must therefore account for the selection of resistant viruses.

The most commonly used method for treating resistant viruses is combination therapy, which uses multiple antivirals in one treatment regimen. This is thought to decrease the likelihood that one mutation could cause antiviral resistance, as the antivirals in the cocktail target different stages of the viral life cycle. This is frequently used in retroviruses like HIV, but a number of studies have demonstrated its effectiveness against influenza A, as well. Viruses can also be screened for resistance to drugs before treatment is started. This minimizes exposure to unnecessary antivirals and ensures that an effective medication is being used. This may improve patient outcomes and could help detect new resistance mutations during routine scanning for known mutants. However, this has not been consistently implemented in treatment facilities at this time.

The success of HBV vaccination programs has been clearly demonstrated over the recent years in several regions around the world. Countries that have adopted the recommendation had a marked reduction in carrier rates as well as complications from HBV, including HCC. The low prevalence of chronic HBV infection in children younger than 5 years, reducing from 4.7% in the prevaccine era to less than 1% in 2019, can be attributed to the widespread use of hepatitis B vaccine. Due to the implementation of routinely birth-dose vaccination the greatest decrease appears to be in the Western Pacific region, from 8.3% HBsAg prevalence in the prevaccine era to 0.93% in 20022015 . Among health care workers, hepatitis B vaccination is highly effective for the prevention of healthcare associated HBV infection and chronic infection. Using mathematical models, it was estimated that since their implementation, HBV vaccination programs have averted 210 million new HBV infections globally .

The Centers For Disease Control And Prevention Universally Recommends The Hepatitis B Vaccine For All Adults Up To Age 59 And For Adults 60 And Over At High

The CDC recommends three actions that healthcare providers should take to implement the new universal recommendation:

  • Offer hepatitis B vaccination to all adults aged 1959 years who have not previously completed vaccination, as well as adults > 60 years with risk factors for hepatitis B or without identified risk factors but seeking protection.

  • Implement standing orders to administer the hepatitis B vaccine as part of routine services to adults who have not completed the vaccine series.

  • Offer hepatitis B vaccination, when feasible, in outreach and other settings in which services are provided to persons at risk for HBV infection .

Because clinical manifestations of hepatitis B are indistinguishable from other causes of viral hepatitis, a definitive diagnosis requires serological testing . Every year in the United States about 2,000 people die following an overwhelming hepatitis B virus infection. In addition, every year about 22,000 people are infected with hepatitis B. Some of them will remain chronically infected, putting them at high risk of the long-term consequences of hepatitis B virus infection: cirrhosis and liver cancer. In fact, with the exception of influenza and COVID-19 viruses, hepatitis B virus causes more severe disease and death in the United States than any other vaccine-preventable disease. On the other hand, the hepatitis B vaccine is an extremely rare cause of a severe allergic reaction called anaphylaxis. To date, no one has died from this reaction, but it is theoretically possible that this could occur.

Because hepatitis B virus is a common cause of severe disease and death in the United States, and because the hepatitis B vaccine does not cause permanent damage or death, the benefits of the hepatitis B vaccine clearly outweigh its risks.

Which Adults Should Be Vaccinated Against Hepatitis B

According to CDC recommendations, adults in the following groups are recommended to receive hepatitis B vaccine:

General

  • All people age 18 years and younger.
  • Anyone 19 years and older who wants to be protected from hepatitis B.

People at risk for infection by sexual exposure

  • Sex partners of people who are hepatitis B surface antigen -positive.
  • Sexually active people who are not in long-term, mutually monogamous relationships.
  • People seeking evaluation or treatment for a sexually transmitted disease.
  • Men who have sex with men.

People at risk for infection by percutaneous or permucosal exposure to blood or body fluids

  • Current or recent illegal injection drug users.
  • Household contacts of people who are HBsAg-positive.
  • Residents and staff of facilities for developmentally challenged people.
  • Healthcare and public safety workers with reasonably anticipated risk for exposure to blood or blood-contaminated body fluids.
  • People with end-stage renal disease, including predialysis, hemo-, peritoneal- and home-dialysis patients.

Others

  • International travelers to regions with intermediate or high levels of endemic HBV infection.
  • People with chronic liver disease.
  • People with HIV infection.
  • People with diabetes who are age 19 through 59 years. For those age 60 and older, clinicians should make a determination of need for
  • vaccination based on their patients’ situation.

International Hepatitis B Vaccine Schedules

*Please note that the first dose should be given as soon as possible. Additional doses require minimum time intervals between doses in order for the vaccine to be effective.

The hepatitis B vaccine is an injection that is generally given in the arm and as a three-dose series. The World Health Organization recommends a 0, 1, and 6-month vaccine schedule, though schedules may vary based on a countrys national immunization program. Completing the hepatitis B vaccine series, preferably beginning at birth, will ensure protection against hepatitis B, hepatitis delta and lower the lifetime risk of liver cancer. Greater than 90% of babies and up to 50% of young children who are not vaccinated and are infected with hepatitis B will have lifelong infection, which makes the birth dose essential to their protection. Please note that the vaccine brand name, manufacturer and associated schedules for adults, children and infants may be unique to different countries, though there is a list of WHO prequalified vaccines.

3-Dose Vaccine Series for Infants

The World Health Organization recommends all infants receive the first dose of the hepatitis B vaccine within 24 hours of birth and to complete the vaccine series with additional shots at 1 month and 6 months of age. Beginning the hepatitis B vaccine at birth will ensure protection against hepatitis B for life.

Who Should Get The Hepatitis A Vaccine

The CDC recommends that all children between ages 12 months and 23 months get this vaccine as well as for any infant aged 6 to 11 months who is traveling internationally.

The following people are also at risk for the disease and should be vaccinated:

  • Children and teens through age 18 who live in states or communities that have made this vaccination routine because of a high rate of disease
  • Men who have sex with men
  • Anyone who uses illegal drugs
  • People with chronic liver disease
  • Anyone treated with blood clotting drugs, such as people with hemophilia
  • People who work with HAV-infected primates or in HAV research laboratories.
  • Travelers to countries where hepatitis A is common. A good source to check is the CDCâs travelersâ health website, which you can search by the country youâre going to.
  • People adopting or close to a child adopted from a country where hepatitis A is common

You should not get the vaccine if you’re allergic to any ingredients in it or if you had a severe allergic reaction to an earlier dose of it. Tell your doctor or pharmacist about any allergies you have.

If you’re pregnant, let your doctor know. The safety of this vaccine for pregnant women is unknown, although the risk is considered to be very low.

Who Should Receive The Hepatitis B Vaccine

Hepatitis B Vaccination Information

For most people, the hepatitis B vaccine is safe and effective. About 90% of people who receive three vaccine doses are protected against hepatitis B for over 30 years.

The CDC’s Advisory Committee on Immunization Practices recommends the hepatitis B vaccine for the following groups:

  • All babies, starting just after birth
  • Children and adolescents under 19 years old
  • Adults ages 1959 who have not previously completed vaccination
  • Adults ages 60 and over with a high risk of contracting HBV

Adults ages 60 and over who do not have any hepatitis B risk factors can receive the hepatitis B vaccine, but it is optional.

Hepatitis B spreads when the bodily fluids of an infected person enter another person’s body. Sexual contact is one way it can be spread. A person with HBV can spread it to their baby during childbirth. Other ways in which HBV may be transmitted include:

  • Sharing medical equipment, whether at home or in a hospital setting, with a person who has an HBV infection
  • Sharing syringes with a person who has hepatitis B, such as during injection drug use or at-home piercing or tattooing
  • Sharing personal items, such as razors or toothbrushes, with someone who has hepatitis B
  • Coming into contact with the sores or blood of a person who has hepatitis B

Routine Administration Schedule For Hepatitis B Vaccine In Adults

  • The dosing schedule is 0, 1 to 2 months, and 4 to 6 months.
  • There is some flexibility in the schedule, but be sure to keep in mind the minimum intervals between doses:
    • At least four weeks between doses #1 and #2
    • At least eight weeks between doses #2 and #3
    • At least 16 weeks between doses #1 and #3
  • If your patient falls behind on the hepatitis B vaccination schedule , continue vaccinating from where your patient left off. The series does NOT need to be restarted.
