Hepatitis C Risk Factors & Symptoms – Frankfort Regional Medical Center

Robert Burakoff, MD, MPH, is board-certified in gastroentrology. He is the vice chair for ambulatory services for the department of medicine at Weill Cornell Medical College in New York, where he is also a professor. He was the founding editor and co-editor in chief of Inflammatory Bowel Diseases.

Some types of viral hepatitis can be transmitted through sexual contact, in addition to being transmitted via other routes. Each type of hepatitis virus has a different risk of being transmitted through sex.

How Can I Protect Myself And Others

  • Never share injecting drug equipment or things that may have blood on them such as toothbrushes and razors. Also avoid sharing straws or rolled up banknotes if snorting drugs with others.
  • Use condoms for anal and vaginal sex and latex gloves for fisting.
  • During group sex, cover anything which goes from one partner to another with a fresh condom or fresh latex glove for each new person it enters. Clean objects with warm water and anti-bacterial soap before using on a new partner.
  • Dont share enema equipment or pots of lubricant.

If you have hepatitis C you may want to tell a partner and explain that youre infectious. They can then decide if they’re happy to take any risks and whether they want to take precautions. That way they cannot accuse you of infecting them without them knowing that the risk was there.

General Transmission Of Hep C

Hep C is most commonly transmitted through blood-to-blood contact, i.e. when the blood of someone with the virus enters the bloodstream of someone else. This can occur through:

  • Sharing needles or syringes or any other drug injecting equipment
  • Unsafe tattooing or body piercing
  • Less common means are from mother to baby at birth , household transmissions and occupational transmission .
  • There is no documented evidence that breastfeeding spreads hep C. If a nursing mothers nipples are cracked and bleeding, she should stop nursing temporarily until her nipples are healed.
  • Before hep C tests were introduced in Australia , some people received contaminated blood transfusions or blood products. All blood donations in Australia are now screened for hepatitis C.
  • Being hep C positive means that you have tested positive with the hep C PCR blood test. Antibody tests cant confirm whether or not you actually have hep C.

In Development: Genital Herpes Vaccines

Genital herpes is a viral infection caused by herpes simplex viruses. Some infected people may have few or no symptoms of illness, but many others experience blisters and sores in the genital area. The infection can remain in the body indefinitely, and sores can recur again and again.

Researchers have developed many experimental attenuated and inactivated herpes vaccines, starting in the 1930s and continuing through the 1970s, though none was effective enough to be approved and licensed.

The National Institute of Allergy and Infectious Diseases and pharmaceutical company GlaxoSmithKline co-sponsored a Phase 3 clinical trial of a candidate subunit herpes vaccine on nearly 8,000 women across the country. The vaccine had previously shown some promise in a certain subset of women. In September 2010, however, researchers reported that the Phase 3 trial failed to show that the vaccine was effective. Another herpes candidate vaccine, sponsored by Sanofi Pasteur, uses the whole virus and is in pre-clinical studies.

As much as it would be useful to have a highly effective herpes simplex vaccine, the current options are not likely to be broadly useful.

Hepatitis and Sexually Transmitted Diseases Explained ...

If you arent sure whether you have hepatitis C, get tested. Testing is especially important if you have sex with more than one person or if you have other risk factors for hepatitis C, including being born being 1945 and 1965, having had a blood transfusion prior to 1992, and injecting drugs .

Talk to your partner about getting tested as well, for hepatitis C and other STDs, so you know the risks before having sex. People who are at risk for hepatitis C are also at risk for HIV and other STDs, emphasizes Talal.

Viral Hepatitis And Sexually Transmitted Diseases

The most common mode of viral transmission is via contact with infected needles as well as contaminated body secretions such as semen, genital fluids, saliva and respiratory droplets. Viral hepatitis is caused by a group pf viruses that holds a special propensity for liver cells. This includes Hepatitis A, B, C and D. Most frequently reported varieties that are transmitted via sexual contract are Hepatitis B and C.

Ner Notification And Preventive Treatment

If tests show that you have an STI, your sex partners including your current partners and any other partners youve had over the last three months to one year need to be informed so that they can get tested. If theyre infected, they can then be treated.

Each state has different requirements, but most states require that certain STIs be reported to the local or state health department. Public health departments often employ trained disease intervention specialists who can help notify partners and refer people for treatment.

Official, confidential partner notification can help limit the spread of STIs, particularly for syphilis and HIV. The practice also steers those at risk toward counseling and the right treatment. And since you can contract some STIs more than once, partner notification reduces your risk of getting reinfected.

What Are The Symptoms Of Hepatitis B

Many people with hepatitis B dont have any symptoms. If you do get symptoms you may not notice them until two or three months after infection and they can last up to three months. There are two types of infection acute and chronic.

Acute symptoms include:

  • jaundice, meaning your skin and the whites of your eyes turn yellow
  • dark urine
  • pale faeces .

People who cant fight off acute infection after six months, such as babies, young children and people with a weakened immune system because of HIV, can go on to develop chronic hepatitis B. This is when people are at higher risk of liver failure, liver disease and cancer of the liver.

What Factors Are Linked To The Sexual Transmission Of Hepatitis C Among Gbmsm

How Is Hepatitis Transmitted?

A cluster of factors have been associated with the sexual transmission of hepatitis C.5,6 For sexual transmission of hepatitis C to occur, there must be exposure to the hepatitis C virus in one or more of these situations:

  • condomless anal sex
  • using recreational drugs before or during sex
  • having HIV
  • having an ulcerative STI

Drug use and sex

Where drug use is present, it is difficult to tease apart the extent to which drug use versus sex contributes to the transmission of hepatitis C. For example, party and play or chemsex is when certain drugs are used by gbMSM before or during sex to facilitate, sustain or enhance sexual encounters. The risk of hepatitis C transmission through party and play is twofold, through drug use and through sex. Hepatitis C can be acquired through shared drug use equipment. There is also concern that booty bumping or boofing can damage and/or irritate the tissue of the rectum, which could increase the risk of sexual transmission of hepatitis C.13 Party and play is also associated with types of sex that overlap with sexual risk factors for hepatitis C. For example, gbMSM who party and play are more likely to participate in condomless anal sex with one or more partners and participate in group sex compared with gbMSM who do not party and play.14 Thus, deciphering whether new hepatitis C infections among gbMSM who party and play were acquired through drug use or sex is challenging.

Can Hepatitis C Be Spread During Vaginal Sex

Its rare for the hepatitis C virus to be transmitted through vaginal intercourse. Unless the vaginal walls arent lubricated or intercourse is very rough and leads to tears in the vaginal wall, theres no opportunity for blood to be exchanged. The risk for transmission with vaginal intercourse is about 1 in 190,000, according to research published in the March 2013 issue of the journal Hepatology.

Research published in the Journal of Coagulation Disorders in March 2014 reinforces these findings, emphasizing that transmission of the hepatitis C virus by sex in monogamous heterosexual couples is rare. And the Centers for Disease Control and Prevention states that couples in monogamous heterosexual relationships do not need to use condoms routinely, even if one partner has hepatitis C. Concerned couples, however, can discuss using a condom to lower the already very low risk of spreading the virus, says Talal.

Just how safe sex is when a partner has hepatitis C hinges on some other factors as well. For example, its important to use a new condom with each sexual act that has the potential to expose the uninfected partner to the infected persons blood, even if youre in a committed relationship, says the CDC. Some of these situations include sex when you or your partner:

  • Has an open cut or sore
  • Has another sexually transmitted infection , especially one that causes sores or lesions
  • Is having her menstrual period

What Is Hepatitis C

The term hepatitis means an inflammation of the liver. The inflammation may damage the liver, which is the largest internal organ in the body. The liver filters the blood and breaks down harmful substances.

Infection with a virus is the most common cause of hepatitis. If a person contracts HCV, the disease is called hepatitis C, or hep C for short.

Hepatitis A and hepatitis B are other types of hepatitis that result from viral infection. Hepatitis A, B, and C differ in the way they spread, their effect on the liver, and their method of treatment, according to the

HCV causes hepatitis C. A person may contract HCV by coming into contact with the blood of someone else who has the condition. Even microscopic amounts of blood may be enough to spread the virus.

In the United States, the way for someone to contract hepatitis C is by sharing drug-injection equipment, such as needles or syringes.

According to the , other ways a person may contract hepatitis C include:

  • coming into contact with the open sores or blood of a person with the condition
  • using the toothbrush, nail clippers, or razor of a person with the condition
  • getting a tattoo or piercing with non-sterile instruments or inks
  • getting an accidental stick with a needle from someone with hepatitis C

About 6% of babies born to people with hepatitis C will develop hepatitis C.

Sometimes an individual will experience symptoms 13 months after contracting HCV, according to

What Are The Chances Of Getting Hep C Sexually

Hepatitis C spreads by contact with an infected persons blood. Although uncommon, hepatitis C can be transmitted through sexual activity, if the person has genital sores and cuts. It can also be transmitted during menstruation. However, just 2% of hepatitis C cases are sexually transmitted.

The risk of getting hepatitis C sexually increases in the following situations:

  • Men who have sex with men

Prevention And The Treatment Potential

Currently, there is no vaccine to prevent HCV infection. However, the recent availability of DAA for the treatment of chronic HCV with cures rates over 95% has created optimism towards HCV elimination. In many countries treatment is now available for all individuals with a chronic HCV infection, irrespective of fibrosis stage . Modelling studies were the first to demonstrate that rapid scale-up of DAA might limit onward transmission and chronic HCV prevalence and incidence among MSM could decline -. However, for substantial reductions a decline in risk behaviour is needed as the scale-up of DAA is counterbalanced by ongoing risk behaviour, resulting in initial and reinfections -. In addition, early treatment, including treatment of acute infection, might further reduce HCV incidence , . As treatment is costly and treatment uptake varies considerably across countries , effective behavioural interventions for MSM at risk of infection are urgently needed. Qualitative research among HIV-positive MSM with a cured HCV infection in the pre-DAA era showed that the strongest motive to implement risk reduction strategies was the reward of avoiding HCV retreatment and its side effects , but this may have changed with the less burdensome DAA treatment. Also sexual risk norms within the MSM population, HCV stigma and non-disclosure of HCV status forms barriers to safer sex, and drug use directly impedes the self-efficacy of MSM to take risk reduction measures .

Contaminated Needles And Infected Blood

You can get hepatitis C from sharing contaminated needles, syringes and other injecting equipment during recreational drug use. Banknotes and straws used for snorting may also pass the virus on.

Being exposed to unsterilised tattoo and body piercing equipment can also pass hepatitis C on. Occasionally, you can get it from sharing a towel, razor blades or a toothbrush if there is infected blood on them.

Hepatitis C infection is also passed on in healthcare settings, from needle stick injuries or from medical and dental equipment that has not been properly sterilised. In countries where blood products are not routinely screened, you can also get hepatitis C by receiving a transfusion of unscreened blood and blood products.

You can prevent hepatitis C by:

  • never sharing needles and syringes or other items that may be contaminated with infected blood
  • only having tattoos, body piercings or acupuncture in a professional setting, where new, sterile needles are used
  • following the standard infection control precautions, if youre working in a healthcare setting.

Sexual Transmission And Hepatitis A

Transmission of hepatitis A virus can occur from any sexual activity with an infected person and is not limited to fecal-oral contact. People who are sexually active are considered at risk for hepatitis A if they are MSM, live with or are having sex with an infected person, or inject drugs. Vaccination is the most effective means of preventing hepatitis A transmission among people at risk for infection. CDC has published recommendations for prevention of hepatitis A that identify all groups recommended for vaccination, including hepatitis A vaccination for MSM.

Because HCV is primarily spread through contact with infected blood, people who inject drugs are at increased risk for HCV infection. HCV can also be transmitted from an infected mother to child at the time of birth, from unregulated tattoos or body piercings, and from sharing personal items that may be contaminated with infected blood, even in amounts too small to see. Much less often, HCV transmission occurs through sexual contact with an HCV-infected partner, especially among people with multiple sex partners and men who have sex with men. Currently in the United States, health care related transmission of HCV is rare, but people can become infected from accidental needle sticks and from breaches in infection control practices in health care facilities.

How Are Hepatitis C And Hiv Connected

People living with HIV are at higher risk for hepatitis C. Of the 1.2 million people living with HIV in the U.S., about 1 in 4 also have hepatitis C.

Having both HIV and hepatitis C means increased risk for liver disease, liver failure and liver-related death from hepatitis C. Because hepatitis is often serious in people living with HIV and may lead to liver damage more quickly, the CDC recommends people living with, or at risk for HIV, also get tested for hepatitis C.

Building Healthy Sexual Relationships

Rule number one for a healthy sexual relationship: Be open and honest with your partners. I believe in transparency, says Talal. This conversation can be difficult, but its important to have. Part of discussing your status is talking about what exposure you may have had to hepatitis C, even in the distant past.

Its a good opportunity for you both to share your sexual history, as well as your experiences with other ways hepatitis C can be transmitted, such as using injection drugs or being exposed to items that may have blood on them, including needles, razors, and toothbrushes.

Sherman explains that even if you consistently use condoms during sex, other activities, such as sharing needles to inject drugs or sharing straws to snort them, increase your risk of spreading hepatitis C. People do not want to hear about this, he says. Its difficult to get the word out about risk.

If you and your partner find that hepatitis C is disrupting your relationship or your sex life, you might also consider working with a marriage and family therapist or a sex therapist.
