How To Treat Hepatitis C Virus

Can A Transplant Cure Hepatitis C

Hepatitis C Virus From Screening to Cure

If you develop chronic hepatitis C and it leads to liver cancer or liver failure, you may need a liver transplant. Hepatitis C is one of the most common reasons for a liver transplant.

A liver transplant removes a damaged liver and replaces it with a healthy one. However, theres a high likelihood that the hepatitis C virus will be transmitted to the new liver in time.

The virus lives in your bloodstream, not just your liver. Removing your liver wont cure the disease.

If you have active hepatitis C, continued damage to your new liver is very likely, especially if hepatitis C remains untreated.

Antiviral Medication For Hepatitis B

Doctors may recommend antiviral medication for people with chronic hepatitis B, which occurs when the virus stays in your body for more than six months.

Antiviral medication prevents the virus from replicating, or creating copies of itself, and may prevent progressive liver damage. Currently available medications can treat hepatitis B with a low risk of serious side effects.

NYU Langone hepatologists and infectious disease specialists prescribe medication when they have determined that without treatment, the hepatitis B virus is very likely to damage the liver over time. People with chronic hepatitis B may need to take antiviral medication for the rest of their lives to prevent liver damage.

There are many different types of antiviral medications available, and your doctor recommends the right type for you based on your symptoms, your overall health, and the results of diagnostic tests. A doctor may take a wait-and-see approach with a person who has a healthy liver and whose blood tests indicate a low viral load, the number of copies of the hepatitis B virus in your bloodstream.

Someone with HIV infection or AIDS may have a weakened immune system and is therefore more likely to develop liver damage. The U.S. Centers for Disease Control and Prevention strongly recommends that people with HIV infection who are diagnosed with hepatitis B immediately begin treatment with antiviral medication.

What Are The Symptoms Of Hepatitis C

#66: Hepatitis C: Workup and Treatment in Primary Care ...

Most people infected with hepatitis C have no symptoms. Some people with an acute hepatitis C infection may have symptoms within 1 to 3 months after they are exposed to the virus. These symptoms may include

If you have chronic hepatitis C, you most likely will have no symptoms until complications develop, which could be decades after you were infected. For this reason, hepatitis C screening is important, even if you have no symptoms.

Antiviral Medication For Hepatitis C

For people with hepatitis C, the goal of treatment with antiviral medication is to prevent the virus from replicating, or copying itself, and to eliminate the virus from the bloodstream. If the hepatitis C virus has been in the body for more than six months, the infection is considered chronic. Without treatment, most people with acute hepatitis C develop the chronic form of the disease.

Your doctor decides which antiviral medicationor combination of medicationsto prescribe based on the results of a blood test called a genotype test. There are six genotypes, or strains, of the hepatitis C virus, and people with certain genotypes respond more quickly to medical treatment.

For many years, the standard treatment for chronic hepatitis C consisted of the antiviral medications pegylated interferon and ribavirin. Ribavirin is taken by mouth every day, and interferon is an injection that you or a caregiver can administer once a week at home.

In 2013 and 2014, the U.S. Food and Drug Administration approved a group of new medications for the treatment of hepatitis C. These medications, which include sofosbuvir, are very effective and have fewer side effects than older medications, particularly interferon.

Evolution Of Hcv Therapy

The ultimate goal of hepatitis C treatment is to reduce the occurrence of end-stage liver disease and its complications, including decompensated cirrhosis, liver transplantation, and HCC. Treatment success is assessed by sustained virologic response , defined by the presence of undetectable HCV RNA in blood several months after completing a course of treatment .

Natural Remedies For Treating Hepatitis C

One of the most disturbing aspects of the disease hepatitis C is that many people carrying the infection dont know they have it . . . until it attacks their liver. That can result in a debilitating condition for the patient because the liver is key to regulating your bodys digestive system and flushing toxins from the body. If left untreated, hepatitis C can eventually lead to death.

Per natural-homeremedies.com, citing statistics from the World Health Organization, between 130 and 150 million people worldwide have been diagnosed as having the disease, with about 350,000 a year dying from it. Its a contagious disease caused by a virus most commonly transmitted through the contact of one infected persons blood with another person, and, less commonly, through transfer of semen or saliva, per healthline.com.

Hepatitis C Symptoms & Treatment

FAST FACTS:

  • Hepatitis C is found in infected blood. It is also rarely found in semen and vaginal fluids.

  • Hepatitis C is mainly passed on through using contaminated needles and syringes or sharing other items with infected blood on them. It can also be passed on through unprotected sex, especially when blood is present.

  • You can prevent hepatitis C by never sharing needles and syringes, practising safer sex, and avoiding unlicensed tattoo parlours and acupuncturists.

  • Hepatitis C will often not have any noticeable symptoms, but a simple blood test carried out by a healthcare professional will show whether you have hepatitis C.

  • In the early stages, some peoples bodies can clear a hepatitis C infection on their own, others may develop chronic hepatitis C and will need to take antiviral treatment to cure the infection.

  • Without treatment, chronic hepatitis C can lead to permanent liver damage.

Hepatitis C is part of a group of hepatitis viruses that attack the liver.

Its mainly passed on through contaminated needles, either from injecting drugs or from needle stick injuries in healthcare settings. It can also be transmitted sexually, especially during anal sex or other types of sex that may involve blood.

Some groups are more at risk of getting hepatitis C than others, including people who use drugs, people in prisons, men who have sex with men, health workers and people living with HIV.

Causes Of Hepatitis C

You can become infected with hepatitis C if you come into contact with the blood of an infected person.

Other bodily fluids can also contain the virus, but blood contains the highest level of it. Just a small trace of blood can cause an infection. At room temperature, it’s thought the virus may be able survive outside the body in patches of dried blood on surfaces for up to several weeks.

The main ways you can become infected with the hepatitis C virus are described below.

What Do Hepatitis C Symptoms Look Like

CDC recommends hepatitis C testing all baby boomers

Hepatitis C infection can go through two stages: acute and chronic. In the early, or acute stage, most people don’t have symptoms. If they do develop symptoms, these can include:

  • flu-like symptoms, tiredness, high temperature and aches and pains
  • loss of appetite
  • tummy pain
  • jaundice, meaning your skin and the whites of your eyes turn yellow

While for some people, the infection will clear without treatment, in most cases, acute infection will develop into long-term chronic infection. Chronic infection may not become apparent for a number of years until the liver displays signs of damage. These symptoms can include:

  • mental confusion and depression these are specific to hepatitis C
  • constantly feeling tired
  • nausea, vomiting or tummy pain
  • dark urine
  • feeling bloated
  • joint and muscle pain

Without treatment, chronic hepatitis C can cause scarring of the liver , which can cause the liver to stop working properly. A small number of people with cirrhosis develop liver cancer and these complications can lead to death. Other than a liver transplant, theres no cure for cirrhosis. However, treatments can help relieve some of the symptoms.

Interferons And Pegylated Interferons

The two most frequently used recombinant interferon preparations in clinical trials have been IFN alfa-2b and IFN alfa-2a , which differ from each other by only a single amino acid residue. IFN alfacon-1 , or consensus IFN, is a genetically engineered compound synthesized by combining the most common amino acid sequences from all 12 naturally occurring IFNs. Roferon-A was discontinued from the market in 2007 and Infergen was discontinued from the market in 2013.

The addition of propylene glycol molecules to IFN has led to the development of long-lasting IFNs that have better sustained absorption, a slower rate of clearance, and a longer half-life than unmodified IFN, which permits more convenient once-weekly dosing. The FDA has approved PEG-IFNs for the treatment of chronic hepatitis C.

Two PEG-IFN preparations are available for the treatment of chronic hepatitis C. PEG-IFN alfa-2b consists of IFN alfa-2b attached to a single 12-kd PEG chain it is excreted by the kidneys. PEG-IFN alfa-2a consists of IFN alfa-2a attached to a 40-kd branched PEG molecule it is metabolized predominantly by the liver.

Treatment Of Hepatitis C In Patients After Liver Transplantation

Chronic HCV infection with end-stage liver disease or hepatocellular carcinoma due to HCV is currently the most common indication for liver transplantation. All transplanted patients become reinfected with HCV and, combined with immune suppressive therapy to avoid rejection of the organ, some patients develop rapid progressive hepatitis. In addition, acute rejection seems more frequent in patients with liver transplantation for hepatitis C.

Before transplantation, the patient should be treated for HCV infection, if possible . Unfortunately, most patients in need of transplantation have decompensated disease, limiting effective antiviral therapy before transplantation. Even when IFN combined with ribavirin was previously used, after transplantation the standard treatment should be PEG-IFN and ribavirin. Nevertheless, SVR rates post-transplantation were significantly worse compared to antiviral treatment in the non-transplantation setting. Overall, SVR rates post-transplantation are only about 20%. Furthermore, in a high percentage of patients, interferon and ribavirin are poorly tolerated after liver transplantation and therapy must be considered carefully.

Medical Treatment For Hepatitis A B & C

Treatment for hepatitis A, B, or C is based on which type of hepatitis is present in the bloodstream and the severity of the resulting liver damage. Depending on the results of diagnostic tests, our specialists at NYU Langone may recommend antiviral medication to stop the virus from replicating and protect your liver from further damage.

Detection Of Viral Core Antigen

Compared to other diagnostic methods like EIA, the advantages of NATs are having higher specificity and sensitivity. However, the disadvantages of these assays are time-consuming and require sophisticated technical equipment, trained technicians, dedicated laboratory space and expensive reagents. In patients with HCV infection, it has been demonstrated that the HCV core antigen level strongly correlates with the HCV RNA level for various genotypes. Thus, due to cheap and easy-to-perform, the HCV core antigen quantification assay can be used as an alternative method to NATs to detect HCV RNA. Currently, core antigen detection by means of a chemiluminescent microparticle immunoassay can be fully automated in the Architect HCV Core antigen test . The Architect HCV Ag assay had a specificity of 100%, with a lower limit of detection of 3 fmol/L corresponds to approximately 1000 IU/mL of HCV RNA. Whereas, current HCV RNA assays have a lower level of detection between 5-15 IU/mL. In general, about 90% of HCV RNA positive samples are positive with a viral load above 10000 IU/mL, well in the sensitivity range of the HCV core antigen assay. Therefore, HCV antigen detection might be the next step following a positive antibody screening test. Several combination assays for detection of both anti-HCV antibodies and HCV core antigen have been developed.
