Our CTC in Cliffwood, New Jersey, serves adults age 18 and older of all genders. Unlike methadone clinics that only offer one type of medication, our CTC can provide patients with methadone, buprenorphine, or naltrexone. These medications allow you to stop using opioids without experiencing the painful withdrawal symptoms that would otherwise occur.

When you start treatment at our CTC in Cliffwood, New Jersey, you will complete a thorough evaluation with a medical professional who can answer all your questions about methadone, buprenorphine, and naltrexone and help you determine which medication is best for you.

Overdose Fatalities Beginning To Decrease

Various news outlets are reporting new statistics which indicate deaths from opioid overdose are beginning to go down.

The Associated Press reports that for the first time in a decade overdoses among New York residents have declined 15.9%. Government officials are quoted as saying that about 80% of the overdose deaths were attributable to heroin or fentanyl.

The AP cited a new CDC July 2019 study which showed overdose deaths in 2018 fell for the first time in nearly three decades.

Various public education efforts and New Yorks Opioid Task Force are thought to be significant catalysts for the slowdown in opioid overdoses. The availability of naloxone has also been highly instrumental in impacting overdoses nationwide with many communities across the country now providing naloxone kits for free.

A number of metro areas in the U.S. are also examining the feasibility of mobile opioid treatment since transportation to clinics or physicians is often an impediment to accessing medication-assisted treatment resources.

Therapies Used At Our Clinic In Cliffwood Nj

Methadone and other medications can be an essential element in treatment for opioid addiction. But medication alone cant help you make the lifestyle changes that will support successful long-term recovery from opioid addiction. This is why our center in Cliffwood, New Jersey, also provides individual and group therapy.

Our therapeutic services are one reason why we refer to our facility as a Comprehensive Treatment Center instead of a methadone clinic.

Individual therapy sessions are confidential, one-on-one discussions with a member of your treatment team. These sessions are ideal opportunities to address important matters you may not yet be comfortable talking about in a group environment.

Group sessions typically involve a small number of adults who are receiving MAT at our center in Cliffwood, New Jersey, along with the professional who is leading the session. Groups are safe spaces where you can share support, gain important information about opioid addiction and recovery, and express your thoughts.

Tolerability And Drug Interactions

In general, antiviral regimens are well tolerated, however there are some nuances. All regimens can potentially interact with concomitant drugs, and it is strongly recommended that potential interactions be assessed and managed. A full review of all prescription, over-the-counter and complementary medicines is essential. The use of a web-based interaction checker is invaluable.8 Because the hepatitis C treatment is relatively short, consider withholding interacting drugs, for example statins .

Sofosbuvir is currently not recommended for patients with an estimated glomerular filtration rate less than 30 mL/min/1.73 m2. Protease inhibitors are contraindicated in patients with decompensated cirrhosis. These patients should be managed by a specialist, rather than by community prescribers. Also those co-infected with hepatitis B should be managed by a specialist as there are concerns about hepatitis B reactivation during hepatitis C treatment. Because of reports of severe hepatitis B reactivation, the US Food and Drug Administration and the Australian Consensus Statement 2 recommend screening all patients for hepatitis B before starting antiviral treatment. The recommended tests are for HBsAg, anti-HBc and anti-HBs. Patients who are HBsAg positive require either concomitant antiviral treatment for hepatitis B, or close monitoring of alanine aminotransferase and hepatitis B DNA during and after treatment.

Summary Of Recommendations For Clinicians And Policy

This recommendation applies to asymptomatic adults who are not at elevated risk for hepatitis C.

It does not apply to pregnant women or adults who are at elevated risk for hepatitis C, including individuals with current or past history of injection drug use, individuals who have been incarcerated, immigrants from hepatitis C endemic regions, individuals who have received health care where there is a lack of universal precautions, recipients of blood transfusions, blood products or organ transplant before 1992 in Canada, hemodialysis patients, individuals who have had needle stick injuries, or individuals who have engaged in other risks sometimes associated with HCV exposure.

The Task Force based the recommendations on the overall balance between the possible benefits and harms of screening, patient preferences and values related to screening as well as considerations related to feasibility, accessibility, resource use and equity.

The Task Force will continue to carefully monitor the scientific and policy developments related to screening and patient access to treatment in Canada.

  • We recommend against screening for HCV in adults who are not at elevated risk.

Subutex And Methadone In Treatment Of Opioid Addiction

Recovery from opioid addiction initially centers around physical stabilization: specifically the management of opioid withdrawal. This is an essential step for the vast majority of opioid addicted people seeking help. Research has shown a 90% failure rate for opioid treatment programs that do not offer medication assistance.

Methadone was the original medication FDA-approved for treating opioid addiction although Subutex has been recently introduced into opioid treatment programs around the country as a viable alternative. Subutex is effective especially for milder levels of opioid dependency.

Subutex is a brand name version of buprenorphine, the partial opioid agonist that reduces withdrawal symptom sickness. Most patients are familiar with Suboxone which is a popular buprenorphine-based film that is dissolved under the tongue and is taken once per day. It differs from Subutex in that it contains naloxone so that it cannot be easily abused intravenously.

A number of methadone clinics began offering subutex in the past few years in an effort to expand treatment options for patients. Because subutex can be abused, it is typically administered daily in the clinic by a nurse where it can be supervised.

While written for clinicians, the guidelines are intended to offer guidance and support to patients as well.

Antiviral Treatment Of Hepatitis C

Multiple oral regimens are currently available on the PBS. They may be prescribed by a medical practitioner experienced in the treatment of chronic hepatitis C infection, or in consultation with a gastroenterologist, hepatologist or infectious diseases physician.

As this therapeutic area is evolving rapidly, it is important that prescribers keep abreast of changes or maintain close links with specialists, to ensure patients are given the most appropriate treatments. It is anticipated that, in the next 12 months, new treatments will be approved that are equally effective across all hepatitis C genotypes. It will therefore become less important to know what genotype the patient has before treatment. In the meantime, it is essential that the hepatitis C genotype is determined, as not all currently approved regimens are effective or available for all genotypes.

Table 1 shows treatment regimens currently approved and available on the PBS. Additional regimens are approved but not recommended as they provide inferior efficacy or tolerability. It should be noted that a short, eight-week course of sofosbuvir/ledipasvir fixed-dose combination is appropriate for treatment-naïve patients with genotype 1 infection, no cirrhosis and a viral load less than 6 x 106 IU/mL. Most other patients require 12 weeks of treatment, while some patients with cirrhosis and previous treatment failure require 24 weeks of treatment.

Screening Diagnosis And Assessment

It is estimated that 82% of the 227 000 people living with hepatitis C in Australia have been diagnosed.4 However, many of these people have either not been informed of their diagnosis or are not aware of the implications of chronic viral hepatitis. Before 2016, fewer than one in four Australians with chronic hepatitis C had been treated and approximately one in five were undiagnosed. Because hepatitis C is a major cause of chronic liver disease, cirrhosis and liver cancer, it is essential that all people with chronic infection are identified so that treatment can be provided.

Anyone at risk of contracting a blood-borne infection should be tested for hepatitis C, as should anyone with evidence of chronic liver disease or abnormal liver enzymes.2 Injecting drug users should be a major focus for testing as they represent approximately 80% of infected people. Other important groups include migrants from high-prevalence countries or regions such as Egypt, Pakistan, Mediterranean and eastern European countries, Africa and Asia.
