Accelerated Us Children And Adult Hepatitis B Vaccine Schedules
*Please note that the first dose should be given as soon as possible. Additional doses require minimum time intervals between doses in order for the vaccine to be effective.
In some instances, it may be necessary to vaccinate within a short period of time to ensure protection before travel. There are accelerated schedules to provide the highest level of protection over a short period of time. Individuals who need an accelerated schedule must have a booster dose at 1 year to ensure long-term protection. Note that the 2-dose Heplisav-B vaccine will also ensure maximum protection over a 1-month period without the need for a booster dose at 1 year.
4-Dose Vaccine Series for Children and Adults
Engerix-B is a 3-dose vaccine that can be given on an accelerated, four-dose schedule, with 3 shots administered within 2 months, and a booster dose at 1 year to provide maximum long-term protection.
4-Dose Combination Hepatitis A and B Vaccine Series
Twinrix is a 4-dose vaccine that can be given on an accelerated schedule to provide protection against hepatitis A and B. Three doses are administered within 1 month, followed by a booster shot at 1 year. This is a common choice of vaccine for those travelling on short-notice outside the U.S. It is important to complete the booster dose at 1 year, to ensure long-term protection.

Who Should Receive The Hepatitis B Vaccine
For most people, the hepatitis B vaccine is safe and effective. About 90% of people who receive three vaccine doses are protected against hepatitis B for over 30 years.
The CDC’s Advisory Committee on Immunization Practices recommends the hepatitis B vaccine for the following groups:
- All babies, starting just after birth
- Children and adolescents under 19 years old
- Adults ages 1959 who have not previously completed vaccination
- Adults ages 60 and over with a high risk of contracting HBV
Adults ages 60 and over who do not have any hepatitis B risk factors can receive the hepatitis B vaccine, but it is optional.
Hepatitis B spreads when the bodily fluids of an infected person enter another person’s body. Sexual contact is one way it can be spread. A person with HBV can spread it to their baby during childbirth. Other ways in which HBV may be transmitted include:
- Sharing medical equipment, whether at home or in a hospital setting, with a person who has an HBV infection
- Sharing syringes with a person who has hepatitis B, such as during injection drug use or at-home piercing or tattooing
- Sharing personal items, such as razors or toothbrushes, with someone who has hepatitis B
- Coming into contact with the sores or blood of a person who has hepatitis B

Does Medicare Cover Vaccines
Medicare covers the flu and pneumococcal vaccines at 100% under Part B, along with the hepatitis B vaccine for those at higher risk. That means no out-of-pocket cost to the patient. A tetanus shot after experiencing a deep wound or burn also should be covered.
Medicare Part D plans should cover the shingles vaccine, but check with your plan. You may have to pay at least a portion out of pocket if you havent yet reached your deductible for the year . There may be a copay, depending on your plan.
Interchangeability And Dosing Schedule
- 2-dose HepB vaccine series only applies when both doses consist of HepB-CpG, administered at least 4 weeks apart.
- Series consisting of a combination of 1 dose of HepB-CpG and a vaccine from a different manufacturer should do the following:
- Adhere to the 3-dose schedule minimum intervals of 4 weeks between dose 1 and 2, 8 weeks between dose 2 and 3, and 16 weeks between dose 1 and 3. However, if HepB-CpG is substituted for dose 2 of HepB-alum, a provider has the option of administering the next dose of HepB-CpG a minimum of 4 weeks from the previous dose for a complete series.

Talk With Your Health Care Provider
Tell your vaccination provider if the person getting the vaccine:
- Has had an allergic reaction after a previous dose of hepatitis B vaccine, or has any severe, life-threatening allergies
In some cases, your health care provider may decide to postpone hepatitis B vaccination until a future visit.
Pregnant or breastfeeding people should be vaccinated if they are at risk for getting hepatitis B. Pregnancy or breastfeeding are not reasons to avoid hepatitis B vaccination.
People with minor illnesses, such as a cold, may be vaccinated. People who are moderately or severely ill should usually wait until they recover before getting hepatitis B vaccine.

New Members Can Get Vaccines At Pharmacies
Effective immediately, members 18 years of age and older with pharmacy benefits can now go to a participating Express Scripts retail pharmacy to receive 11 different immunizations. Vaccines are covered with no cost share to the member and a prescription is not required. Previously, only the flu vaccine was available from a retail pharmacy.

In Massachusetts, the pharmacist must be registered and licensed to administer the vaccine. Since requirements vary in other states, members should contact their local pharmacy to confirm availability.
Your employees can continue to get their immunizations at their doctor’s office or at an outpatient clinic. The addition of the pharmacy option is intended to expand access to these preventive services. Employees should contact their pharmacy directly to confirm if a pharmacist is available to administer any of these vaccines.
This benefit applies to all Access Blue, HMO, PPO, POS, and indemnity plans with pharmacy benefits.

Who Should Get Vaccinated
The Centers for Disease Control and Prevention recommends:
- Birth through 6 years: DTaP in a total of 5 doses: at 2 months, 3 months and 6 months, again at 15 through 18 months, and once between 4 and 6 years. Total of 5 doses.
- 7 through 10 years: If not fully vaccinated against pertussis, should receive a single dose of Tdap OR, in some cases, should be vaccinated according to the catch-up schedule with Tdap as the preferred first dose.
- 11 through 18 years: Tdap as a single dose preferably between 11 and 12 years. If not fully vaccinated, check catch-up schedule. For adolescents 13 through 18 who missed getting Tdap at 11 through 12, administer at soonest opportunity.
- 9 years and older: Anyone who did not receive a dose of Tdap should get one as soon as possible.
- Pregnant Women: Should get a dose of Tdap preferably at 27 through 36 weeks gestation.
What Does Masshealth Cover For Flu Vaccines
MassHealth pays for administration of flu vaccines when LPHDs and PSDs provide MDPH-supplied flu vaccines to eligible MassHealth members up to and including 18 years of age. In addition, MassHealth pays LPHDs for purchasing and administering flu vaccines for eligible MassHealth members 19 years of age and older. Hepatitis is an inflammation of the liver, most often caused by a viral infection. There are three common types of hepatitis caused by viruses: hepatitis A, hepatitis B, and hepatitis C. Vaccines have been developed that protect people from contracting hepatitis A and B. There is no vaccine for hepatitis C.
Hepatitis A and hepatitis B can be spread from person to person, although in different ways. They have similar symptoms, which include abdominal pain, fever, fatigue, joint pain, and jaundice .

Sbp Adjuvant For Hepatitis B Vaccine
Wang and colleagues stated that although adjuvants are a common component of many vaccines, there are few adjuvants licensed for use in humans due to concerns about their toxic effects. There is a need to develop new and safe adjuvants, because some existing vaccines have low immunogenicity among certain patient groups. A good laboratory practice, non-clinical safety evaluation was performed to identify the side effects of both SBP and SBP-adjuvanted hepatitis B vaccine. The results indicated that SBP could enhance the HBsAg-specific immune response, thus increasing the protection provided by the hepatitis B vaccine. The authors concluded that given the encouraging safety data obtained in this study, further evaluation of SBP as a vaccine adjuvant for human use is warranted. They stated that this research has the potential to accelerate adjuvant development for HBV vaccine and for other vaccine types in the future.
