Abstract

1. Introduction

Approximately, 71 million people are infected with hepatitis C virus worldwide . Pakistan in the South Asian region has a high burden of active HCV infection . The new directacting antiviral agents have changed HCV treatment landscape leading to cure rates of over 90% . Furthermore, the pan-genotypic DAAs have shown promising results in complicated patients and have reduced the pill burden . Nevertheless, during the last few years, an increasing number of reports suggest the availability of complementary and alternative medicines as a natural remedy in the use of HCV treatment . However, user and CAM provider perspectives in the use of CAM in the treatment of HCV have not been explored before. The term CAM is used collectively for all those remedies that are not included in the western allopathic system . National Center for Complementary and Alternative Medicine defines CAM as a group of diverse medical and health care systems, practices, and products that are not presently considered to be part of conventional medicine . CAM uses natural health products such as vitamins, homeopathic remedies, herbal medicines, spiritual healing, chiropractic remedies, and prayers .

Then, the extracts without cytotoxicity were screened for their anti-HBV activity. Four of the 41 extracts suppressed extracellular HBV production in HepAD38.7 cells by > 25% . Since the maoto extract suppressed HBV production the most , we focused on the maoto extract for further investigation.

    Suzuki in 1983 published the results of a randomized, controlled, double-blind study with 133 chronic hepatitis patients who were given either SNMC or placebo for 4 weeks. Both ALT and AST levels were lower in the SNMC treated group when compared to the placebo group throughout the entire treatment period however, within 2 weeks of follow-up, there was no longer a statistical difference in aminotransferases between the 2 groups. Also, HCV RNA remained present throughout treatment in both groups. In addition, this study did not distinguish between patients with hepatitis B and hepatitis C. These findings suggest that SNMC results in a decline in aminotransferase levels in patients with chronic liver disease, but that it has no lasting benefit once therapy is stopped.

    Cyclophilin As An Anti

    Cell culture-based screening for anti-HCV compounds identified cyclosporin A as a potential agent. CsA, produced by the fungus Tolypocladium inflatum Gams, is known for its immunosuppressive effects and organ transplant application. The responsible target for CsA anti-HCV activity was cyclophilin PB . CyPB is a cellular peptidyl prolyl cis-trans isomerase. It is mainly localized at the cytoplasmic site of the ER membrane, exactly like the NS5B polymerase of HCV. Furthermore, both components form complexes with HCVRNA. Specific knockdown experiments showed that CyPB stimulated the RNA binding activity performed by NS5B, and a lack of CyPB led to reduced replication, respectively. Since CsA disrupts the complex of CyPB and NS5B, as a consequence, HCV genome replication is less efficient. Due to its immunosuppressive activities, CsA is not suitable as an anti-HCV drug. However, NIM811, a derivative with only 1 amino acid substitution, completely lacks the immunosuppressive effect and even provides approximately 2-fold stronger binding affinity to CyPB. Clinical trials are now underway. The fact that the CsA treatment of patients with HCV recurrence after liver transplantation, who did not respond to HCV standard therapy, resulted in a decrease of HCVRNA below the detection level in 5 of 8 patients, gives hope for positive results of cyclosporins.

    Honokiol, a lignan isolated from leaves of Magnolia officiais, was shown to have multiple effects on HCV infection, strongly inhibiting HCV infection in both the J6/JFH1 reporter virus system and in a pseudo-particles system containing glycoprotein of three different HCV genotypes . The mechanism in which honokiol affects HCV entry was reported by Lan et al. . The treatment of cells with this natural compound suppressed the OCLN and SR-BI receptors involved in HCV infection. The authors also demonstrated that honokiol was able to inhibit HCV IRES-mediated translation. However, this antiviral action was observed only at the highest concentration treatment and no effect was detected at concentration lower than 20 M. Honokiol was also effective against HCV replication as observed against two SGR, the genotype 1b and genotype 2a .

    Silymarin is a mixture of silybin A, silybin B, isosilybin A, isosilybin B, silychristin, isosilychristin, and silydianin extracted from Silybum marianum . This extract has shown recently to block virus entry, RNA and protein expression, virus production and cell to cell spread of virus . One of the major components of silymarin is silibinin, a mixture of semi-purified silybin A and silybin B and commercially available.

    There was also a significant difference in the use of herbal medicine according to the type of hepatitis with which patients had been diagnosed. Patients who were diagnosed with hepatitis C and those diagnosed with other forms of hepatitis including alcoholic hepatitis and drug-induced hepatitis were more likely to use herbal therapies compared to those who were diagnosed with hepatitis B before adjusting for other factors.
