    Sue and associates stated that autochthonous HEV infection has been reported in over 200 solid organ transplant recipients since 2006, yet little is known about the burden of HEV among SOT recipients in North America. In a retrospective, single-center, cross-sectional study, these investigators examined the prevalence and risk factors associated with HEV infection among SOT recipients at their institution. Children and adults who received allografts between 1988 and 2012 at the Johns Hopkins Hospital were assessed for evidence of HEV infection by testing post-transplantation serum samples for HEV antibody by enzyme immunoassay and HEV RNA by reverse transcription quantitative polymerase chain reaction . Individuals with evidence of post-transplant HEV infection were compared with individuals without evidence of infection and assessed for risk factors associated with infection. A total of 12 individuals developed post-transplant HEV infection. Post-transplant HEV infection was associated with an increased risk for graft rejection . No individuals developed chronic infection. The authors concluded that SOT recipients in the United States are at risk for post-transplant HEV infection. Moreover, they stated that a prospective, multi-center, study is needed to supplement the findings of this retrospective, single-center study and to better understand HEV infection among SOT recipients in North America.

    Study Design And Data Source

    We conducted a retrospective study using results in the NHI Lab & Exam Dataset as the reference standard and linked them to the NHI Outpatient Claims Dataset to assess the performance of ICD-10-CM codes in identifying patients with HBV and HCV infections. This study was approved by the Institute of Review Board of National Cheng Kung University Hospital with record number B-ER-107-394.

Men were less likely to have had an HCV test, compared with women . Patients aged 2029 years had a higher HCV testing rate , compared with other age groups. Patients who had a diagnosis of drug poisoning, overdose, or suicide attempt by drug misuse or CPT code for MAT were more likely to have been tested for HCV, compared with patients without those conditions. Patients with an infection indicative of IDU were less likely to have been tested for HCV .

    What Spurred This New Hcv Screening Recommendation

    The new B recommendation represents an update and expansion of the USPSTFs 2013 recommendation that clinicians screen for HCV in high-risk patients and offer one-time testing to adults born between 1945 and 1965. Reasons for expanded screening are two-fold:

    • New evidence shows a dramatic rise in the number of new HCV cases per year, now approximately 3.5 times higher than it was 10 years ago, and
    • Better treatments for HCV are currently available that have improved safety and efficacy.

    The broadening of the HCV epidemic is largely attributable to a rapid increase in rates among younger people particularly women, IV drug users, and those who live in rural areas. And HCV infections in individuals older than 79 years are expected to increase as baby boomers age. The recommendation statement brings to light the troubling reality that HCV infects one-third of IV drug users aged 18 to 30 that HCV prevalence among women aged 15 to 44 years doubled from 2006 to 2014 and a 68 percent increase occurred in the proportion of infants born to HCV-infected mothers in recent years.

    New Recommendations For Hcv Screening

    HIV ICD-10-CM Codes – When to Use Z Code vs. B Code

    To update its previous recommendation, the USPSTF commissioned a systematic evidence review that included nearly 100 studies. The new review was similar in scope to the previous one except that it also examined evidence on adolescents. The evidence collected led the USPSTF to issue a B recommendation for HCV screening in all adults aged 18-79 years in the draft statement. The task force also suggests that physicians consider screening patients for HCV who are aged younger than 18 years and older than 79 years if they are at high risk for infection.The USPSTF is accepting comments on the draft recommendation statement and draft evidence review on screening adults for HCV infection until Sept. 23. The task force will consider all comments received as they prepare the final recommendation.
