Guidance On Reporting Adverse Events Following Immunization

Vaccine providers are asked to report, through local public health officials, any serious or unexpected adverse event temporally related to vaccination. An unexpected AEFI is an event that is not listed in available product information but may be due to the immunization, or a change in the frequency of a known AEFI.

Refer to Reporting Adverse Events Following Immunization in Canada and Adverse events following immunization in Part 2 for additional information about AEFI reporting.

Every Oregonian age 12 and up is eligible for a vaccine. For Washington County-sponsored clinics, 12 to 14-year-olds must be accompanied by either a parent, guardian or someone designated by the parent. If someone other than a parent or guardian accompanies the 12 to 14-year-old, they will need to provide proof of parental/guardian consent. For many other clinics, those ages 12-14 do not need to be accompanied by an adult if they have the signed consent form.

Proof of consent is either:

  • A signed consent form ” rel=”nofollow”> English and on the site)
  • A written or typed note that includes the parent/guardians name, relationship to the young adult, their date of birth, a statement saying they consent to young adult being vaccinated and the parent/guardian signature.

Fifteen and older do not need to be accompanied and do not require parental consent in the state of Oregon.

Is A Booster Required For Hep A

The hepatitis A infection is rarely fatal, but it can cause debilitating symptoms and acute liver failure if not treated, according to the World Health Organization.

O’Reilly said he was left feeling confused about whether or not a booster shot is in fact required to protect against hepatitis A.

A single dose of the vaccine “usually provides good protection” and a single dose is “appropriate post-exposure protection against a known exposure event,” Ottawa Public Health said in a statement to CBC News.

“But to ensure protection against future exposures over many years, it is best to get a second dose six months or more following the initial dose.”

Customer Told Booster Was Necessary

“The pharmacist who gave us the shot emphasized, I think more than once during the session, that it was very important that we come back for the second shot which would be provided free of charge by Costco,” said O’Reilly.

“There was absolutely no ambiguity about this.”

O’Reilly said he and his wife felt “tremendous” anxiety at the time because they know for certain they consumed the recalled berries, so he made a point of jotting down the six-month mark in his calendar.

There was absolutely no ambiguity about this.- Dan O’Reilly

But O’Reilly said when he contacted the same Costco on Thursday, he was told by the pharmacist that Costco had only committed to providing the first shot.

“The pharmacist started going on about their commitment was just to provide the first shot, and that the full vaccine was really not their responsibility, and on, and on, and on. And we’re going, ‘Wait a minute, you committed to providing two shots, which is the full vaccine,’ and the pharmacist basically said, ‘Well that’s not what we’ve been told.'”

La County: Possible Hepatitis A Exposure At Montebello Costco

The Los Angeles County Department of Public Health is working closely with Montebello Costco Wholesale to alert consumers of a possible low risk of hepatitis A exposure associated with butchered meat and seafood products that may have been contaminated by a worker with hepatitis A. This hepatitis A case does not appear to be related to the current hepatitis outbreak among homeless. Out of an abundance of caution, Costco and Public Health warn consumers that anyone who handled and or consumed butchered meat and seafood from Costco Wholesale located at 1345 N Montebello Blvd, Montebello, CA 90640, during the period of October 22 through October 31, 2017, may be at low risk for hepatitis A. This possible exposure does not affect butchered meat and seafood products purchased from other Costco locations.

Public Health is working closely with Costco Wholesale to investigate this possible exposure and to provide at-risk Costco employees with hepatitis A vaccination as needed. Costco has notified their member customers who have bought these products. There is no known risk of hepatitis A exposure at the Montebello Costco Wholesale now or on other dates outside the exposure period, and Public Health has not identified any new cases associated with this possible exposure.

Costco will offer free hepatitis A vaccinations at the Montebello Costco for any customers who may have handled or consumed the meat and seafood products from October 22 through October 31. When:

Closing The Vaccination Equity Gap

Washington County is partnering with more than 70 community organizations to close the COVID-19 vaccination equity gap. Our COVID-19 , submitted to the state on September 28, outlines the progress we have made in the last four months since our first in May 2021. This includes significant progress vaccinating Black and Latina/o/x adults. However, there is still work to do as vaccination rates in these groups remain significantly lower than in other racial/ethnic groups in the county.

Washington County was the first county in the state to vaccinate 80% of all adults. Governor Kate Brown congratulated us and partner during a September 22 visit to the mobile vaccination clinic at the Forest Grove Farmers Market. See and governors news release in and .

Costco’s Response ‘doesn’t Seem Right’

Hepatitis A vaccines are not funded by OHIP, except to individuals who are considered high risk, such as people with liver disease, men who have sex with other men, and intravenous drug users, according to Ottawa Public Health.

Still, O’Reilly said “it doesn’t seem right” that Costco promised something to its customers that it now says it won’t deliver.

“The original message was very clear and anybody who had planned on that is obviously now being put in a position where they have to fall on their own resources to complete a process that they were told wasn’t discretionary, you had to go get it,” said O’Reilly.

A Costco Canada spokesman confirmed to CBC News that the company did in fact tell customers in April that boosters would be necessary, and would be provided for free.

Costco stores offer Hepatitis A shots after recall, Report ...

This article was published more than 5 years ago. Some information may no longer be current.

Costco is offering Hepatitis A vaccinations to anyone who purchased a frozen berry mix recently recalled by the Canadian Food Inspection Agency.

The CFIA recalled Nature’s Touch Organic Berry Cherry Blend on Saturday due to possible contamination. The frozen berry mix is sold exclusively at Costco stores in Ontario, Quebec, New Brunswick, Nova Scotia, and Newfoundland and Labrador.

The Public Health Agency of Canada says 13 cases of Hepatitis A have been linked to the product nine in Ontario, three in Quebec, and one in Newfoundland and Labrador with the individuals becoming sick in February and March. It says three people were hospitalized.

The health agency says anyone exposed to the recalled product, or have symptoms of Hepatitis A, should see their health care provider immediately.

The PHAC says Costco Canada is now offering free Hepatitis A vaccinations for anyone who has been affected by the recall. The agency says vaccinations can prevent the onset of symptoms if given within two weeks of exposure.

Vaccinations will be available at most Costco warehouses, the company said in a communiqué.

The public-health agency says Hepatitis A can cause inflammation of the liver, and sufferers can experience a wide range of symptoms including fever, low appetite, nausea, vomiting, diarrhea, muscle aches, and jaundice.

    Costco Covid Vaccine Information

Our favorite wholesale retailer Costco is now helping deliver the Covid vaccine in the U.S and weve gathered all the information for you. Please keep in mind, for most Americans its going to be a while before you can get it. However, were excited that Costco is now vaccinating people and it will be coming to more states soon! There are a lot of logistical hurdles but were glad Costco is doing its part in helping solve this complicated problem.

There are a few reasons that contribute to the delay in Costco administering the vaccine. The most obvious is that the vaccine is not getting distributed at a pace we all wish for. Secondly, most Costco pharmacies do not have the cold-storage freezer required for the Covid vaccine. For example, the flu shot can be stored at 40° Fahrenheit, which is the same as a standard household refrigerator. The Pfizer Covid vaccine on the other hand, requires a storage temperature of -94° Fahrenheit. Yes, that is a 134° difference between cold and extremely cold!

We spoke with our local Costco pharmacy in the San Francisco Bay Area and the freezer is the current hold-up. They also explained that the Costco pharmacies have been put into different tiers that decide when theyll get their freezers installed. For our local Costco, that means mid-March will be when the freezer will likely get installed.

Reversal Based On Health Agency Guidelines

But after the Public Health Agency of Canada issued new guidelines indicating the booster shots are not necessary, Costco reversed its decision to provide them, Ron Damiani said.

In the days immediately after the recall Costco was working with “all kinds of sources,” including healthcare units throughout Ontario, Quebec and the Maritimes,” Damiani said.

“At the time that we did it, we were told to tell people that a booster shot would be required in six months,” said Damiani.

Damiani said customers who still want a booster will have to make arrangements through their own health-care providers.

Why Is It So Important For Seniors To Get Flu Shots

Who is at the greatest risk from catching the flu ?

    • children less than 5 yrs old
    • individuals suffering from certain existing health conditions

The statistics for the elderly with regards to the flu are tough to read

    • 90% of all flu-related deaths in the US are people aged over 65 years old
    • 60% of flu-related hospital stays in the US are people over 65 years old

Getting a flu shot done each year is one of the best measures available to prevent and protect against this.

Flu shots are available in a high strength dose which is usually recommended for the elderly.

The source for the statistics is: NCOA Center for Benefits Access Vaccines: What Medicare pays forwhich you can find and read here.

With each year an elderly person becomes more susceptible to catch the flu virus if they come into in contact with it.

And, the older an individual is, if they catch the flu, the greater the chance is that they may go on to develop a serious complication, such as pneumonia.

In addition, any underlying health conditions can increase the risk of even greater complications from the flu virus, and sadly the elderly are the demographic most likely to have underlying health conditions, just because of their age.

Underlying conditions which can increase the risk of greater complications from the flu virus include

    • Tetanus, Diptheria, Whooping Cough

Costco Gives Free Hepatitis A Shots In Outbreak Tied To Fairview Processor

In an usual move, Costco is reimburses or giving free hepatitis A shots to people who purchased Townsend Farms Organic Antioxidant Blend. The mix was tied to an outbreak that’s stretched so far to seven states.

Costco is offering free hepatitis A shots in the outbreak traced to an Oregon berry processor.

A company spokesman said Thursday that Costco will reimburse the price of vaccine or administer shots for free to customers who purchased Organic Antioxidant Blend of frozen fruit, produced by Townsend Farms in Fairview

The hepatitis A vaccine costs about $120 per dose, and usually two doses are needed.

The announcement comes as the outbreak linked to the berry and pomegranate blend continues to grow. So far no cases have been identified in Oregon.

    More

Costco’s move is very unusual, said Bill Marler, a food safety litigator in Seattle. Although other retailers have volunteered upfront to pay for medical costs in connection with a manufacturer’s product, it’s a rare practice, he said.

Craig Wilson, food safety manager for Costco, said the company decided to help health departments burdened with requests for shots.

“It’s the right thing for us to do,” Wilson said. “We’re happy to help.”

Costco will cover the price for both doses, Wilson said.

    Why You Should Take Note

The Public Health Agency of Canada has collaborated with federal and provincial public health partners to investigate an outbreak of Hepatitis A infections in three provinces linked to the frozen fruit product: Natures Touch Organic Berry Cherry Blend. On April 15, 2016, the Canadian Food Inspection Agency issued a food recall warning advising Canadians of the recall of a frozen fruit product that had been distributed in Ontario, Quebec, New Brunswick, Nova Scotia, and Newfoundland and Labrador. Given that the source of the outbreak was identified and contaminated products have been recalled from the market the outbreak investigation coordinating committee has been deactivated and the investigation is coming to a close.

Although the outbreak appears to be over, it is possible that additional cases of Hepatitis A related to this outbreak could be reported in the future given the long incubation period for Hepatitis A. The overall risk to Canadians is low. Hepatitis A is a disease that can cause inflammation of the liver. Hepatitis A can range in severity from a mild illness lasting a few weeks to a severe illness lasting several months. You can get the Hepatitis A virus by eating contaminated food or water or through contact with an infected person’s stool. Adequate vaccination can protect against the Hepatitis A virus.
