A Traditional Chinese Medicine Maoto Suppresses Hepatitis B Virus Production

Worldwide, millions of people suffer from hepatitis B virus infection, putting them at a high risk of death from liver cirrhosis and cancer. Although effective anti-HBV drugs have been developed, current drugs have some limitations, as most of them have a risk of significant side effects. Therefore, the discovery of safe and effective anti-HBV drugs is still needed. Natural compounds are considered sources of novel, safe and effective therapeutics. In this study, we screened a library of Kampos, traditional herbal medicines, for suppression of HBV production. Among them, we found that maoto reduced extracellular HBV DNA but not extracellular HBsAg during HBV infection, suggesting that it suppressed HBV production by interfering with HBV nucleocapsid incorporation into viral particles. Furthermore, we revealed that maoto reduced the expression of a host gene, Tropomyosin chain , whose downregulation also suppressed HBV production, similarly to maoto. Since the safety of maoto has been already confirmed, maoto can be considered a candidate anti-HBV agent if the effect is confirmed in vivo. In addition, our findings also suggest TPM2 as a novel molecular target for the development of anti-HBV agents.

Results Of One Chinese Clinical Trial
In a clinical trial carried out at three test sites in China, there were 94 inpatients treated and evaluated, utilizing three therapies . The main test therapy was the formula now called Salvia/Ligustrum Tablets, taken with the vitamin tablet. The primary control therapy was a tablet of the same size and same dosage made with lentinus extract , atractylodes, and schizandra the control patients also took the vitamin supplement. The third control group took a patent medicine made in China that was understood, by the Chinese researchers, to be the most effective one available.
As a result of 12 weeks treatment, in the Salvia/Ligustrum group and the Lentinus group there was marked improvement in symptoms and liver enzymes, more so than with the Chinese patent remedy. Antigen conversion from positive to negative occurred for HBeAg in 2/3 of patients treated by Salvia/Ligustrum or Lentinus Tablets, but in only 1/3 of those receiving the Chinese patent. HBsAg conversion occurred only in a few patients, 2 receiving Salvia/Ligustrum, 1 receiving Lentinus, and none receiving the Chinese patent. Nearly half of those tested showed viral DNA conversion from positive to negative in those receiving Salvia/Ligustrum, and in one-third of those receiving Lentinus, but only in about one-fourth of those receiving the Chinese patent.
Filipino Doctors And Hep C Treatment
Nowadays a lot of my work is helping people in poor countries, such as the Philippines and Nigeria, to access Hepatitis C treatment. One of the barriers that I come up against is that many doctors in these countries do not understand Hepatitis C or its treatment.
Lately I have had a number of examples of doctors from poorer countries prescribing totally the wrong medication for the treatment of Hep C.

Silybum And Its Component Silymarin
Silymarin is a complex flavonoid from Silybum marianum, an herb that was initially introduced as a therapeutic agent by European researchers, but soon taken up by their counterparts in China as a treatment for liver diseases. Its main action is to protect the liver from damage, and it is used, for example, in the early stage of liver destruction due to ingestion of poisonous mushrooms, to save the lives of victims. A concentrated extract of silybum, rich in silymarin, is sold as a health product in the U.S. and the isolate has been sold as a drug in Europe for the treatment of liver disease for the past 15 years. A dosage of 140 mg/day is reported to be liver protective and a dosage of 420 mg/day is reported to help repair liver damage. It is not clear that silymarin can cure viral hepatitis. Silybum extracts are widely sold as health foods in the United States.

Ligustrum And Its Component Oleanolic Acid

Akapulko Or Ringworm Bush Or Acapulco
Also known as Bayabas-Bayabasan, Akapulko is used to treat tinea infections, insect bites, ringworms, eczema, scabies, and itchiness. The leaves can be pounded and its extract directly applied to the affected skin problem. The leaves contain chrysophanic acid, a fungicide that is used to treat fungal or skin infections. The leaves extract can also be used in homemade soaps, shampoos, and lotions.

Transient Hepatitis B Virus Production
HepG2 cells were cotransfected with pHBI together with pGL3 to normalize the transfection efficiency and an shRNA-expressing plasmid using GenJet In Vitro DNA Transfection Reagent according to the manufacturers instructions. At 7 days post-transfection, the culture supernatant was used to measure the level of HBV production by real-time PCR, and cell lysates were used to check the transfection efficiency by a Luciferase Assay System .

Study Design And Setting
The study was conducted within the gastroenterology clinic of Mulago hospital, a tertiary teaching and National Referral Hospital, in Kampala which handles up to 100 adult patients weekly on average with either luminal gastrointestinal issues or liver/pancreatic conditions. The study specifically targeted patients who had a known diagnosis of liver inflammation due to different causes such as hepatitis B virus, hepatitis C virus, drug-induced and alcohol-induced inflammation as shown on their medical records. Data was collected using standardized questionnaires followed by focus group discussions on a sample of patients. The questionnaires used were modified from the International Questionnaire to Measure Use of Complementary and Alternative Medicine .
European Commission And Thervacb Join Forces
The role of viral hepatitis as a public health threat has long been underestimated. Only very recently, the United Nations in their 2030 Agenda for Sustainable Development called for international action to combat viral hepatitis and reduce the disease burden. The major killer is the hepatitis B virus causing liver cirrhosis and liver cancer. Worldwide 880,000 humans die each year from the consequences of an HBV infection.
A prophylactic vaccine is available to prevent HBV infection, but more than 3% of the worlds population are chronically infected and do not profit from that vaccine anymore. For those suffering from chronic hepatitis B, until today no curative treatment option exists.
The European Commission therefore selected the project TherVacB led by Helmholtz Zentrum München for a five-year funding within the Horizon 2020 program. A consortium of leading virologists, immunologists and physicians specialized in treating viral hepatitis, will use a newly designed therapeutic vaccine, TherVacB, as an immunotherapy to cure HBV. TherVacB will be evaluated in a three-year clinical trial starting in 2022 conducted in Europe and in Africa. Integration of a partner site in Tanzania shall help building local capacities for diagnosing and treating hepatitis B and support an important goal of the consortium to raise awareness for hepatitis B.

How Is It Currently Treated
There is no cure for chronic hepatitis B virus.
In most cases, treatment requires taking a pill every day for life to remain effective and to reduce the risk of liver cancer. Even then, it doesnt eliminate the risk.
Chronic hepatitis B hasnt been cured so far in part because current therapies have failed to destroy the viral reservoir, where the virus hides in the cell.
This is in contrast to hepatitis C virus, which has no such viral reservoir and can now be cured with as little as 12 weeks of treatment.

Despite the huge human and economic toll of chronic hepatitis B, research to cure the disease remains underfunded. There is a misconception that because there is a vaccine, hepatitis B is no longer a problem.
The availability of effective cures for the unrelated hepatitis C virus has also led people to believe that viral hepatitis is no longer a problem.
Experts estimate that liver cancer deaths will substantially increase in coming decades without a cure for hepatitis B, despite deaths from most cancers decreasing.
